My wife who has been severly depressed for over three years has just started taking remeron (she was on prozac and is still on effexor). The first two times she has taken it, it has knocked her on her ass. She immediately needs to go to bed and the next morning, she is terrible tired and can hardly get out of bed.

Yess, Remeron does a real number on you at first...when I started I was very lethargic. I started on 1 pill at bedtime, and it took over a week to have that effect ease up. Then I increased to 2, and again, was knocked on my ass. once more, that lasted 1-2 weeks tops. My pdoc says I can increase to 3, which (judging from my mood the past 3 weeks I likely need to do) but it is pretty tough dealing with the tiredness, so I am hesitant... other than that, I have found no other side effects, except perhaps my appetite. I seem to always have an appetite, even when my stomach is sending out "I'm full" messages. my bf was taking Remeron for a while some time ago, right after it came out, and he had lots of side effects....agitation, and the like. I don't have that. It has helped me, though...I am one who becomes resistant to meds over time, so I have been on a lot of different ad meds, and this is one I have to say is better than others. Well, as they say YMMV. But I had the same effects from Remeron. It made me dizzy and just literally kicked my ass. I stayed with it for about a month, with little improvement. So I asked my doctor to let me try something else. I also gained 35 pounds in the month I took it, and I never had that kind of rapid weight gain in my life. It's important that your wife communicate all the side affects and mood changes to her doctor. Finding the right AD is simply a trial and error deal. I had some very strange agitation at first with Remeron...I would jump like I had been hit with a jolt of electricity when the phone rang or any unexpected loud noise. But that effect wore off after a few weeks. Also, I take it only at bedtime, and after three months now, I find that the sleepiness has disappeared. However, the positive effects are less, also. The good thing about Remeron is that it does help take the edge off of the depression and seems to give me some sense of *control* over my daily choices and how I react to stressful situations. As others have mentioned in their replies, my sweet tooth craving is absurd. I have gained 24 pounds. On the other hand, I had lost about 30 pounds from not eating (depression-related) before I began the drug. I have stopped the weight gain by taking an over-the-counter diet pill twice a day. This has the added benefit of making me more alert. My pdoc has approved the diet pill, but your wife should check with her own doc before trying the combination. This is particularly important if she has high blood pressure or anxiety-related racing heart (PET) which can be aggravated by the *speed* effect of the diet pill. I have also been put on Buspar (last 3 weeks) 2/day, which has gotten me to the point that I can manage to handle getting out of the house and going to work. Tell her to be patient and work closely with her pdoc and things will gradually improve. These less-potent drugs are not quick-fixes, but they do make the journey easier.
