Question:
My wife was prescribed Zispin (Mirtazapine) in September last year to control her serious depression. Excellent result at first - it improved her appetite and generally she felt much better. Suddenly a week before Christmas she started feeling anxious about people and eating. This has now become a major (and I mean MAJOR) problem.

Answer:
I tried Zispin (mirtazepine) in the hope that it would be better than Prozac. Actually I've tried it twice now and should have learnt my lesson the first time. Basically a similar story to your wife. I slept really well and felt really great for about four days, and I was eating like a horse. The experience peaked after the fifth and then I started to become really anxious and have unpleasant uncharacteristic thoughts. The slightest thing would irritate me and then I just began feeling listless and empty. I had to take 2 weeks off work because of these odd side effects. I carried on taking the tablets for another week and a half and by this time, I felt awful, really 'fluey' and my nose was permanently congested. I then went back to the doctor who promptly returned me to Prozac. When I did some research on this 'new' drug, I found out that it is very similar in structure to Mianserin, also a product of the Dutch pharmaceuticals company Organon. When you look at the two drugs and compare their side effects profile they have a curious similarity with one serious though rare effect of causing depression in white blood cell count. This is worth considering when taking either of these drugs and doctors used to insist on a full blood count on a monthly basis for anyone taking Mianserin. It is a shame that in some cases, patients seem to know more about their medication than their doctors do. The main effects seemed to wear off after about 3 days into withdrawing the drug. I had started on 15mg and worked up to 30 mg. I stopped abruptly and suffered headaches and sneezing attacks for about a day afterwards. The stuffy nose wore off after about the 3rd day. I started to feel 'normal' again after the 4th day. With any psychotropic drug, particularly ones that are soluble in fat, they can take quite a long time to completely work their way out of your system, though the half life is the main factor, which I think is fairly long, though not as long as Prozac which takes several days to reach half the level in your bloodstream that you were at when your stopped it. Zispin was raved about on daytime TV as a new wonder drug that worked more quickly than the other antidepressants. I agree that it works quickly to alleviate the depression, but that the unwanted effects that soon develop make it unsuitable and unpleasant for some people. I would be interested in hearing other stories. Has your wife tried St. John's Wort? I find this excellent though I cannot tolerate the dietary restrictions that you should follow when taking any MAOI including no red wine or strong cheese!
